Who can we contact in an emergency?

Your Personal Health Information and your Medical Records may be collected, utilised and disclosed, including but not limited to, the following reasons:

• Administrative purposes in running this practice (including billing purposes and compliance with Medicare requirements)
• Facilitating the exchange of relevant information with other healthcare providers involved in your care (eg., healthcare providers within this practice, specialists, allied health professionals)
• For follow up reminder/recall notices for treatment or preventative healthcare Complying with any legislative or regulatory, as mandated by applicable laws (e.g., infectious disease reporting)
• Utilising your data for research purposes while ensuring that it is de-identified, meaning that your identity cannot be discerned from the provided information
• Accessing previous pathology and radiology results for reference and continuity of care

It is your right to withdraw, alter or restrict my consent at any time by notifying this practice in writing.

Our Practice also sends information to the Australian Childhood Immunisation Register and Pap Smear Register. These registers also send reminders, which can be helpful if you move
